There are many reasons definitely, some are burden that was hard to circumvent like her private e-mail server. Others are campaign tactics.
Noticeably, she campaigned in the wrong place in the last week prior to the election. Like in Arizona where she will not be able by any stretch win the electoral votes. She would have don better to campaign in the mid-west states where toss up between her and Donald Trump.

Emotions are very important for human behavior. Clearly, Hillary Clinton ignored this fact. One of the strongest human traits is being aroused and angered and seeking resolution. The trait considered is anger and being linked with the trait of being victimized. Donald Trump played that to the fullest by claiming that America being victimized by unfair trade practices and agreements like NAFTA and alike. Hillary, did not speak about that or make any promises for the American voters. She kept the field open for Donald Trump whether he will deliver on the promises to the voters or they will be bubble soaps that will soon burst. Hillary should have promised fair trade to satisfy the American voters.

There is strong human desire for change and new. As such she did not offer a change but continuation of Barack Obama whether good or bad. She should have a new picture and new future image.
The opposite Trump did just that and promised new America number one. Whether he will deliver on the promises or not? Since he won more than two months ago he has setbacks but he won.

Hillary Clinton private computer server was a constant problem. Besides its criminal aspect and controversy. The shadow of the mistrust accusation hanged over her shoulders and was always used against her by her detractors and very probably got her to loose many votes. How many, tens of thousands or more, no body knows.

As late October month surprise FBI Director James B. Comey made crazy comments that Hillary Clinton is under new investigation regarding the private server at residence. This gained lot of coverage in the media and was circulated around. Trump took full advantage of such new accusations in his fiery rhetoric campaign rallies. Comey retracted two days before the election day November 8, 2016 from such accusation but was low key and hardly gained notice.
5- Speculative Guess
One may suggest if Hillary Clinton had chosen Berni Sanders rather than Tim Kaine as a running mate she would have won the Midwestern states that Sanders have beaten her badly during the primaries and accordingly she would have won the elections over Donald Trump.
